Please see page 4 of the application for instructions. Once the questionaires have been
completed, please mail them and send to Mount Angel Seminary Admissions Office.

Health Form - attachment B
Please have form completed by your Doctor, then mail it to Mount Angel Seminary
Admissions Office.

Psychological Assessment - attachment C
This form includes recommended dimensions for the psychological assessment. It is
preferred to have your assessment completed in your first language, then translated in English.
When this is complete, send it to Mount Angel Seminary Admissions Office.

This is required for students who are not residents of the United States and who will be seeking
a student visa.

This document is for non-native English speaking applicants. It gives the requirements for
admissions to the seminary.

All documents are in PDF format.
(You may fill this out as much as possible, save a copy to your computer. If you are
not able to complete the form at one time, you may print a copy and type or print the
remaining sections, then mail the completed application material to Mount Angel
Seminary Admissions Office.)
